Terms of E-mail Communication
Authorization to use E-mail for Classes & Support
I hereby authorize Northwestern Memorial Hospital to communicate with me or with an authorized person on my behalf via electronic mail (e-mail) regarding any classes, programs or events I am registered for or take part in. If I choose to have information communicated with an authorized person on my behalf, I will provide the authorized person's e-mail address.
I acknowledge and understand that e-mail messages sent to me are not encrypted and travel over the Internet, resulting in a risk that any e-mail will be intercepted and ready by third parties to whom the e-mail is not directed. I hereby assume the foregoing risk.
By agreeing to accept e-mail communication, I understand that I am also authorizing Northwestern Memorial to retain my e-mail address in its databases so that it may send me future communications regarding its services, fund raising activities and other Northwestern Memorial business matters. Additionally, I understand that I may opt to e-mail information from the system to a third party by accepting these terms.
I understand that I may revoke this authorization at any time by providing written notice, via e-mail or otherwise, to webinquiry@nmh.org. I acknowledge that Northwestern Memorial will not sell, transfer or otherwise disclose my e-mail address or any of my other personal information to any third parties without my prior consent.
English is my primary spoken and written language and I fully understand the meaning of this authorization.
